The champ had the belt, with his name inside it. Official contenders were awarded a key. When they challenged for the belt, they either got to unlock the belt and put their own name in it, or they gave the key over to the champ. The original belt had those keys dangling from it that the champ had collected, but I'm not clear if the newer one does or not.
